How To Enroll

How to Apply (New) or Enroll (Current)?

Below please find the process to apply for admission to St. Mary School System.  If you have any questions about this process please do not hesitate to contact us at 270-442-1681.

Students Currently Enrolled:

·         For students already enrolled this school year, an email will be sent for each child in school with a verification link.  When you click on the student verification link, it will allow you to check your information, make any changes, add medical and emergency information for that student, delete any incorrect information, and lastly check yes/no/maybe that your child is returning to St. Mary for the 2017-18 school year. 

·         Each student’s data needs to be verified which is why you may receive more than one email.  For example, if you have three children at St. Mary, you will receive three (3) emails, one for each child, for information to be verified and to register that child.  The entire process should take 3-5 minutes.

All K-12 Students:

·         The $50 Registration Fee per student must be submitted to the school office. This fee will be reduced from first month’s tuition but will hold your place for the upcoming school year.

·         It is the responsibility of Catholic families to contact their parish if they plan to utilize subsidy to assist with tuition.  The parish and the St. Mary Finance Director will communicate about enrolled students and ensure that the parish subsidy is applied to your child’s tuition bill.  If you have questions about how to access parish subsidy or additional financial assistance, please contact our System Director, Eleanor Spry, at 270-442-1681, ext. 273.

New Preschool students:

·         The $50 Registration Fee per student must be submitted to the school office. This fee holds your place for the upcoming school year.

·         All new preschool students must complete a Registration Application and a Preschool Day/Time Form.   A new student is one that did not attend St. Mary during the 2016-17 school year.  The application and form may be completed on-line or paper copies are available in the elementary school office. Paper copies should be submitted to the school office.

·         Copy of Birth Certificate

·         Copy of Social Security Card

·         Current Kentucky Immunization Form

Incoming Kindergarten Students or New Students in Kentucky for the first time:

·         All new students need to complete a Registration Application.  A new student is one that did not attend St. Mary during the 2016-17 school year. The application can be completed on-line or paper copies are available in elementary, middle or high school offices. Paper forms should be submitted to the school office.  Once the application has been received, the student and their family will be notified of admission status.

·         Copy of Birth Certificate (if not already on file from preschool)

·         Copy of Social Security Card (if not already on file from preschool)

·         Current Kentucky Immunization Form

·         Kentucky Medical Examination Form: This physical may be obtained anytime within one (1) year prior to entry into kindergarten.

·         Kentucky Eye Examination From: This exam may be obtained anytime within one (1) year prior to entry into kindergarten.

·         Kentucky Dental Screening/Exam Form (For Kindergarten only): This screening/exam should be completed by January 1st of the first year of school enrollment (January 2017).

 

Incoming New Students (grades 1-12)

·         All new students must complete a Registration Application.  The application may be completed on-line or paper copies are available in elementary, middle/ high school offices. Paper forms should be submitted to the school office. Once the application has been received, the student and their family will be notified of admission status. 

·         Student records from previous school will be requested by St. Mary School System.

 

Incoming 6th Grade Students

·     Current Kentucky Immunization form which includes the following:

o   Tdap Vaccine, regardless of first dose of this immunization

o   Menigococcal Vaccine

o   Varicella Vaccine (Chicken Pox) Vaccine: second dose or Physician documentation the child has had this disease.

o   6th Grade Physical Exam on KDE approved form.  This is different than a KHSAA Athletic Physical, which cannot be accepted as the 6th grade physical.

Athletic Physicals

·         Kentucky High School Athletic Association (KHSAA) requires the use of the appropriate form, either high school form or middle school form.  Middle school uses the middle school form unless they are play on a high school team.  The form must be turned into the school office before the student participates on any athletic team sponsored by St. Mary Schools.  The sports physicals are good for one year and are separate and in addition to any other required documents.  Please click here for link to Athletic Physical forms.
